Subject: Failing UTAK PM Plus Low control
Good morning,
In June, we ordered UTAK PM Plus Low urine control. I recently put the lot in use and noticed that it was failing our quality control parameters after reconstitution. Thinking that there was an internal mistake, I reconstituted a second vial. Both are failing our quality control check and this is something that we have never had occur with this particular QC. I looked at the verified values on the product sheet and the analytes that are failing also have a very low verified value. It is lot D5362 and I am including a copy of the Product description sheet with highlights on the analytes that are failing.
Is there any way that we could request a new lot (at no cost) of this quality control that is closer to the expected value? My other alternative would be to make some changes to our internal expected values for this lot, which is a bit more involved. I wanted to ask to see if a switch out would be an acceptable remedy before doing that.
I'm happy to send back the unused portion, but it is failing our acceptability parameters.
