8/27/2025 Dear Account Manager: I am writing to inform you that on 8/25/25, Quest Diagnostics completed a Full Laboratory Management Agreement with Corewell Health. The transition planning is now under way and NDA has been lifted, therefore, moving forward, we can begin our discussions regarding integrating the Corewell Health laboratories to our supplier base and purchasing operations with Quest Diagnostics standards. The conversion date for new Corewell Health service contracts will be as of their Phased Go-Live dates on the next page. On these dates, Quest Diagnostics acquires the responsibility of purchasing and maintaining service contract coverage moving forward. As a supplier to Quest Diagnostics and in accordance with the terms of our agreements with your company, Quest Diagnostics is requesting that you add all corresponding Corewell Health accounts to your database of locations included in the Quest Diagnostics contracts as of the following expected effective date at the bottom of this letter so that all shipments henceforward will be invoiced at the Quest Diagnostics price, if it is the most advantageous contracted pricing. If the Corewell Health System pricing is more favorable than the prices set forth in our agreements with your organization, please contact your Quest Diagnostics Category Manager for the next steps. Please also make sure that you notify your appropriate Quest Category Manager of any pre-existing contracts and commitments this laboratory has in place with your company.
