We do have your billing address as the same as you provided. We also show an email to send invoices to as: debrakirk77@gmail.com. If there is a better email to do or a preference for a physically mailed invoice let us know. Otherwise we will send it to the billing email on file by email. Once you get the invoice you can use the credit card for payment.

2. Confirm shipping address: 2020 8th Ave., Ste. 218, West Linn, OR 97068 (this is also the billing address)
3. Confirm you want to purchase 1 ea of our parts 17010, 17011, 17012, 17013 and 17014. (One of each level of our validity QC’s) We do want to purchase 1 each of the 5 levels of Specimen Validity Quality Controls, part numbers 17010, 17011, 17012, 17013 and 17014
