As a condition of doing business with the City of Phoenix, you are required to provide a Certificate of Insurance (COI), pursuant to the Insurance and Indemnification section of your contract. The Certificate of Insurance must include the City Contract number in the Description of Operations and List the City of Phoenix as the Certificate Holder. Failure to do so may be considered non-compliance with the terms of your contract which may result in delayed payments or corrective action up to and including contract termination. Additionally, per the attached contract (Section IV – Insurance and Indemnification, page 23), please ensure that coverage for Products – COMP/OP AGG is included under Commercial General Liability. Please submit an updated COI by responding to this email no later than December 3, 2025. (See attachments for reference.) Thank you!
